Purpose:   The purpose of this form is to gather as much information as possible about your wedding or rehearsal dinner speech as I can so that I can write the best presentation possible at the least cost to you.

Please Relax.   This form is just a place to get started. In almost every case, I will interview you by telephone to get the details I will need. This form just gives me a good grasp of the full engagement so I can help you focus your thoughts in our phone conversation. If you don't mind, I'll record our phone conversation.

Answer What You Can:   While I'd appreciate it if you would try to supply as many details as possible, I recognize that, in many cases, you may not know some information or not have an opinion and that's OK. Section VI: Wedding Party

Name the known members of the wedding party that you want mentioned in the speech. I've included a few titles of people typically involved in wedding parties. Remove the one's that do not apply and add as many as you need. State the persons position and then their name.

Identify the topic of the desired speech and any guidelines that need to be incorporated. Any details you can specify will help create a better customized speech.
